He is survived by his mother, Jennifer (Michael) Dempsey, Mount Dora, FL; father, John Michael Polk, Orlando, FL; 3 brothers, Marc Polk, Mount Dora, FL, Clayton Polk, Mount Dora, FL, Jackson Polk, Mount Dora, FL; 2 sisters, Javelyn Dempsey, Mount Dora, FL, Joscelyn Dempsey, Mount Dora, FL; 3 grandparents, Dolores Rochow, Patricia Keller, & Frank Keller. A Celebration of Nicholas' Life will be held at First Baptist Church, Eustis on Sunday, December 4, 2011 at 2:00 PM.
